Abstract

While childhood immunization has been a cornerstone of public health in India, adult immunization remains under-prioritized and given lesser attention. With increasing life expectancy, urbanization, and prevalence of non-communicable diseases, the need for protecting adults from vaccine-preventable diseases (VPDs) is becoming more critical than ever. This paper discusses the rationale for adult immunization in India, assesses its current status, identifies key challenges, and suggests strategies to strengthen adult vaccine uptake in the country. Evidence suggests that adult vaccination significantly reduces morbidity, mortality, and healthcare expenditures, while also improving productivity and quality of life. Despite these benefits, India lacks a universal adult immunization programme, and current recommendations from national advisory bodies remain poorly implemented due to limited awareness, high out-of-pocket costs, weak surveillance systems, and inadequate integration with primary healthcare services. Building upon the experiences from the COVID-19 vaccination campaign, the article highlights policy, system, and implementation-level actions required to institutionalize adult immunization as an integral component of India’s preventive health framework. With strategic investment and sustained political commitment, adult immunization can become a key pillar of preventive healthcare, contributing to healthier ageing and improved population health outcomes.
